Need A Family Practice is a provincial list for people without a family practice (family doctor or nurse practitioner).

Please complete the form below and click the submit link to send us your information. You will be added to a list and contacted directly by a family practice when a spot becomes available. You will need a Nova Scotia health card number, as well as your civic address.

In some cases, you will be registering someone other than yourself for this service. If so, please have their information ready so you can complete the application on their behalf.

If you have registered in the past with Nova Scotia Health Authority, your information has automatically been entered into our new system. If you need to update your personal information (i.e. new address or phone number) please call 811, Monday to Friday between the hours of 10 a.m. and 6 p.m.


The personal health information you voluntarily provide on this form is collected, used and shared to find you a primary health care provider. We follow Nova Scotia’s Personal Health Information Act (PHIA) to ensure your personal health information is protected. Any personal health information you provide through this site will be handled and stored securely to protect it from unauthorized access or disclosure.
